A contact form that actually goes
somewhere useful

An embeddable contact form that sends submissions to your AppTriage inbox — not a separate email thread. Messages land right next to App Store and Google Play reviews, with browser info, timezone, and category attached.

The problem

Your support email is a black hole

Contact form submissions go to support@ and get buried in email threads. No structure, no metadata, no way to tag or triage. Meanwhile, app reviews live in a completely different silo. Two feedback channels, zero connection between them.

Typical contact form
User submits a message on your support page
Email lands in support@ alongside newsletters and spam
No device info, no browser version, no page URL
No connection to the same user's App Store review
Message gets lost in reply-all threads
AppTriage contact form
User submits a message on your support page
Submission lands in your inbox with browser, timezone, category tag
Appears right next to App Store reviews from the same app
Tag it, set a status, reply by email — all from one place

Installation

One iframe, zero configuration

<iframe src="https://apptriage.com/f/your-form/" width="100%" height="600" frameborder="0"> </iframe>

Copy this from your form settings. Paste it into any HTML page. That's the entire integration.

  • Minimal theme blends with any site design
  • Collects browser, timezone, language automatically
  • Category dropdown for routing — Bug, Feature Request, General, Other
  • Email field for direct reply from your inbox
  • Works on any page — support, contact, help center

Why AppTriage

Not just another contact form

Google Forms and Typeform send submissions to a spreadsheet or a separate inbox. AppTriage puts contact form submissions in the same place as your App Store and Google Play reviews.

  • One inbox for ALL feedback channels
  • Tag, triage, and track with status workflow
  • Reply by email directly from the inbox
  • AI auto-categorization on Pro plan
  • Also available as a popup widget, in-app form, or QR code

Common questions

Everything about the embeddable contact form. Can't find an answer?

Get in touch
Is the contact form free?
Yes. The free plan includes 1 app, 100 feedback items per month, the embeddable contact form, the popup widget, and email replies. No credit card required. No trial that expires. If you need more apps or higher volume, Pro starts at $19/month.
How do I embed it on my website?
One iframe tag. Copy the embed code from your form settings in the AppTriage dashboard and paste it into any HTML page — your support page, contact page, help center, or anywhere else. It works on any site that supports HTML iframes, including WordPress, Squarespace, Wix, and static HTML.
What's the difference between this and the popup widget?
The contact form is embedded inline on your page — visible without clicking anything. The popup widget is a floating button that opens a feedback overlay when clicked. Use the contact form for dedicated support or contact pages where you want the form front and center. Use the widget for collecting feedback on every page of your site without taking up layout space.
Does it collect device information?
Yes. The form automatically captures browser name and version, operating system, screen resolution, timezone, language, country, and the page URL where the submission was made. This metadata appears on each feedback item in your inbox — no extra configuration needed. We do not store IP addresses. See our Privacy Policy for full details.
Can I customize the form design?
Yes. The minimal theme removes borders and the accent bar so the form blends seamlessly with your page. You can also set a custom accent color, change the heading text, toggle fields on or off, and choose between default and minimal themes. All configured in your dashboard — no code changes needed. See customization options on the live demo.
Where do submissions go?
Your AppTriage inbox, alongside App Store and Google Play reviews. Every submission appears as a feedback item you can filter, tag, triage, and reply to — all in one place. If the user shared their email, you can respond directly from the inbox. You can also set up notifications to get alerted on new submissions.
